This program will be held at Watermark Place, 10001 Windstream Dr. across from Columbia Mall.

Parking at Watermark: Parking is on the upper level. Entrance to the building is through the main entrance on the lower level. The doorman will let you in and tell you where to go (room is in the entrance level to the left of the lobby.

This is the third session of a series and focuses on Advance Directives. You need not have attended previous events. A lawyer will be present to help answer more specific questions about Advance Directives and help individuals complete a more detailed Advance Directive, or collect the named agent form.  Participants will have an opportunity to scan your Advance Directive into HCGH Database and have an account. An HCGH Wellness Center expert will be available to facilitate entries into the registry. TVIH members register online.
